They'll also need verification of your disability which prevents you from
reading books such as learning disabilities and or visual impairments or
total blindness.
You will need to have your doctor fill out one of the worksheets giving the
information.
That's usually the biggest piece of information they need and once you
provide that with the rest, they should approve you quickly.

        They will need your NLS number to prove you qualify. Also, an
earlier post mentioned $50 a year, but there is also a $25 first time set up
charge, so your first year will be $75, but considering the number of books
you can get, 'Cheap at the price' doesn't even come close.
